Welcome to Orchard Natural Medicine! We want to make your new patient experience is as pleasant and seamless as possible. That’s why you can schedule your first appointment online at your convenience.
Scheduling Your First Appointment
Our practice utilizes the Jane app which will allow you to create a patient account, schedule appointments and fill out all new patient paperwork online once your first appointment has been scheduled.
The first step is to book your appointment using the button below. You are also welcome to call us at (630) 962-6073 to schedule your first appointment.
Once you’ve completed booking your appointment, you’ll get an email with an invitation to create a patient account as well as an email to fill out our online intake form. Our comprehensive intake form will take you about 20 minutes to complete and will provide us with the information we need to better understand your case.
While it is preferred that you fill out all our intake forms online, you can also download them by using the link below.
Regardless of the method you use please fill out our intake forms before you come in for your first appointment.
The new patient onboarding process involves two visits. We encourage you to wear something comfortable because you will be getting a physical exam during your first visit.
The First Visit
During this initial appointment, Dr. Perrin will get your complete health history, perform a physical exam, and complete blood labs (if needed). Please plan on up to approximately 75 minutes for the initial visit during which time she will discuss financials with you.
The Second Visit
When you return we will discuss your treatment plan and review any potential labs that have been done. This visit and future follow-up appointments are typically 30 minutes.
Payment Options
We accept Blue Cross Blue Shield (BCBS) health insurance, participating as a basic Preferred Provider Organization (PPO) as well as BlueChoice PPO. BCBS patients are responsible for any co-pays, co-insurance, and deductibles at time of service.
In regards to other health insurances, Dr. Perrin will submit a claim to be processed under your out of network benefits. Cash, check, debit, credit cards, CareCredit® and Health Savings Accounts (HSA) are also accepted. Payment is due at the time of service for all out-of-network insurance patients and cash patients.
